About Intraoperative Neurophysiology
Intraoperative neurophysiology (ION), also known as intraoperative neuromonitoring, intraoperative
neurophysiologic monitoring ( IONM) , or neurophysiologic intraoperative monitoring (NIOM), employs
electrophysiologic techniques that enable reliable visualization of nervous system functions during brain, spine, vascular, or cardiac surgeries. When performed accurately, ION techniques help identify, map, and/or continuously monitor critical nervous system components at risk of injury, allowing early detection and reversal of dysfunction. This real-time information must be delivered promptly and integrated into the multidisciplinary efforts in the operating room, enabling surgical and anesthesia teams to make swift adjustments to prevent permanent neurological damage.
The ultimate goal is to reduce the risk of postoperative deficits affecting motor, sensory, bowel, walking, speech, vision, hearing, swallowing, or breathing functions.
In particular, ION plays a crucial role in neuromapping and neuromonitoring during spine surgeries, providing real-time tracking of neural function to prevent severe complications such as paralysis. Its use has become
increasingly standard in specific spine procedures; however, this rise has also led to a surge in malpractice claims related to its misuse or omission, highlighting the importance of proper application and
